Description
i was going to rendevous, targeted vessel, i was about 150km distance around mun while burning to retrograde of target and viewing map.
No idea how to do this again.

Updated by I have been having more CTD issues with 1.11 and 1.12. No idea how to replicate either. Happens in different situations

Log also ends with "CheckEncounter: failed to find any intercepts at all" spam before crashing.
My method is different and seems repeatable.

Are you in Jool orbit or one of its moons? This area seems to be dangerous to game stability. I have a save right now that crashes as soon as I load the ship which is in orbit of Jool with no maneuver set. The last line of my log says:MissionsApp does not execute in this game mode, destroying this instance
I tried using KML to fix my persistent file but this ship wasn't one of the ones needing fixing. The common problem with my other ships is docking port states. To see the crash I am experiencing, unpack and load in the attached persistent file, go to the tracking station and filter for stations only. The last one in the list is my Jool Explorer. Switch to that and you will see the crash within a second or two of loading.

If I am in orbit of Jool outside of range of the outer most moon Pol KSP has the tendency to crash to desktop.
But no issues if I am inside the Pol orbit of Jool.
