HI!
In orbit, an offset gizmo appeared. Note that i have not been in EVA build mode during this flight, tho i have been on EVA.

Not only offset, but also rotate. Didn't do any construction either, just got out of the capsule.

Affects 1.11.1 as well.

Gizmo appeared after Bob (unable to do EVA construction) returned from EVA. No mods or DLC were installed.

Possibly related to bug #27442, seems to persist in 1.12.3
